LTA Psychiatric Wellness Center Terms & Conditions

1. Acceptance of Terms

By visiting this website, booking services, or receiving care from LTA Psychiatric Wellness Center, you agree to these Terms & Conditions, our Privacy Practices, and related policies. If you do not agree, please do not continue to use this website or seek services. 

2.Services Provided
We offer outpatient psychiatric and mental health services, including: 

• Psychiatric evaluations 
• Medication management 
• Psychotherapy and supportive counseling 
• Telehealth services 
• Coordination with other providers 

We do not provide emergency care. In the case of a crisis, call 911, go to the nearest ER, or dial 988. 

3.Appointments & Cancellations

• Appointments must be scheduled in advance. 
• 48 hours’ notice is required for cancellations. 
• Missed or late-canceled appointments may result in a cancellation fee as outlined in your service agreement. 

4.Payment & Billing

• Payment is due at the time of service unless otherwise arranged. 
• You are responsible for all out-of-pocket costs, co-pays, or uncovered services. 
• A Good Faith Estimate is available upon request in compliance with the No Surprises Act. 

5.Controlled Substance Policy

If controlled medications (including stimulants) are prescribed, a Controlled Substance Agreement must be reviewed and signed. This includes rules about use, safety, refills, and monitoring. Misuse may result in discontinuation of the prescription or services. 

6.Telehealth Services

Telehealth services are available when appropriate. You agree to: 

• Be physically located in a state where your provider is licensed 
• Ensure privacy on your end 
• Accept the possibility of technical interruptions 

 Telehealth follows the same privacy and clinical standards as in-person care. 

7.Confidentiality

Your health information is protected by HIPAA. We do not share your information without your consent except as required or permitted by law.

8.Communication Consent

You may receive messages by phone, text, voicemail, or email related to your care, scheduling, or billing. You can opt out of non-essential communication at any time by notifying us in writing.

9.Technology Use

We may use HIPAA-compliant digital tools such as secure Telehealth platforms, electronic health records, and AI-assisted scribe technology to support documentation and care delivery. Consent will be obtained if applicable. 

10. Right to Refuse or Discontinue Services

We reserve the right to discontinue care based on clinical judgment, treatment non-compliance, policy violations, or inappropriate conduct. Patients also have the right to withdraw from care at any time.
